Artificial intelligence is no longer limited to generating insights or supporting analysis. With every passing day, AI systems are being designed to initiate actions, trigger workflows, and influence outcomes with minimal human intervention.

This transition to agentic AI represents a significant transformation in how risk is perceived within organizations. It also reveals a governance issue that numerous organizations are not yet prepared to address.

From decision support to decision influence

Organizations referred to AI as a decision-support tool for an extended period of time. Models generated suggestions; however, the ultimate authority was held by humans, this way clear accountability was evident every step of the way.

However, lately this narrative has been complicated by the use of agentic systems. Industry research now characterizes AI systems as something that continuously monitors environments, reasons over live data, and initiates actions within defined boundaries, rather than merely reporting risks upward.

McKinsey has characterized this shift as one that materially changes the governance and risk profile of AI by increasing autonomy while compressing response. In tightly integrated environments, agentic systems can shape outcomes before a human has time to intervene. Especially for this reason, risk is no longer solely derived from inaccurate outputs, but from how autonomy is granted, constrained, and overseen.

Yet many governance models still assume a human decision-maker at the center of every material action.

When accountability becomes unclear

As AI systems gain autonomy, accountability often becomes diffused rather than clarified. Controls are overseen by security squads, policies are administered by compliance teams, AI-enabled tools are implemented by business divisions, models and platforms are supplied by vendors, and risks are evaluated at a high level by oversight committees.

Hence, when an agentic system contributes to harm, responsibility is frequently fragmented across these various functions. Despite the fact that each group may have acted within its mandate, no single group can provide a comprehensive explanation for the final outcome.

This ambiguity is not theoretical. Governance Intelligence has observed that boards are becoming more concerned about the inadequacy of conventional governance mechanisms, including periodic reviews, risk registers, and policies, in managing AI systems that operate autonomously and evolve continuously, according to a Diligent report, “How AI Will Redefine Compliance, Risk and Governance in 2026, Governance Intelligence.”

The system behaved as designed” becomes an explanation rather than an assurance.

Regulation assumes ownership

Regulatory frameworks are beginning to reflect this reality. Even when AI systems operate with a degree of autonomy, the EU AI Act establishes expectations regarding accountability, human supervision, and traceability for AI system outcomes.

The legal analysis of the Act has demonstrated that agentic AI will be fully integrated into its risk-based governance model, particularly in cases where systems influence decisions or actions with legal or operational implications, according to CMS Law-Now, Agentic AI and the EU AI Act.

Similarly, the NIST AI Risk Management Framework and ISO/IEC 42001 prioritize governance over the behavior of AI systems throughout their lifecycle, rather than solely their intended purpose.

These frameworks assume that organizations can pinpoint where autonomy exists, who authorizes it, and who remains accountable when outcomes deviate from expectations. Many organizations cannot yet do so with confidence.

The governance blind spot

Traditional governance focuses on intent; on the flip side, Agentic AI demands governance of behavior. Although policies may strictly prohibit specific applications of AI, systems may still indirectly influence outcomes by prioritizing, automating, or orchestrating workflows.

Risk assessments may authorize a model’s use case without considering the downstream dependencies of its outputs. Many professional services firms, including KPMG, have warned that agentic AI today requires a fundamental reevaluation of accountability, highlighting the necessity of governance being integrated into operating models rather than added on top.

The gap between governance assumptions and operational reality widens as autonomy increases.

Autonomy does not eliminate responsibility

One persistent misconception about agentic AI is that automation reduces accountability. In reality, it raises the stakes further.

When humans act, intent and judgment can be examined; however, when systems act, accountability must be designed way in advance. Without clear ownership, organizations are left explaining outcomes after the fact rather than governing them proactively.

This is not a technical malfunction. It is a failure of governance.

Agentic systems require explicit decisions regarding the extent to which autonomy is permissible, the guardrails that limit it, the circumstances in which human intervention is necessary, and the manner in which actions are recorded and evaluated. In the absence of these decisions, autonomy becomes accidental rather than intentional.

Risk at machine speed

Agentic AI also changes the tempo of risk. Decisions that previously took days to develop can now be made in a matter of seconds. Due to this reason, errors are transmitted at a much quicker pace, and dependencies accumulate at an unprecedented rate. Today, traditional escalation models struggle to keep up with this pace. The Thomson Reuters Institute has observed that agentic AI introduces governance and security challenges due to its ability to operate across systems and processes at a velocity that exceeds the capacity of conventional oversight mechanisms. 

This does not imply that organizations must eliminate autonomy; it simply means that they must proactively manage it.

Reframing the Conversation

The question organizations should be asking is not whether agentic AI is coming; it is already here. The more critical question is whether governance models are evolving at the same rate as needed. Accountability cannot be retrofitted after autonomy is deployed. It must be embedded into system design, operational workflows, and organizational roles. Ignoring these questions does not delay risk; it concentrates on it.

When agentic systems fail without clear accountability, organizations face regulatory inquiries, reputational damage, and most of all internal confusion. Trust erodes not because AI was used, but because governance could not explain its use. Until accountability evolves alongside autonomy, agentic AI will continue to expose a governance gap that no amount of policy language can close.
